BPJS in Indonesia: What Is It, Who Is Eligible, and How Does It Work?
Understanding Indonesia’s National Social Security System

BPJS (Badan Penyelenggara Jaminan Sosial), or the Indonesian Social Security Agency is a government-mandated institution that provides affordable and accessible social protection for all citizens and residents in Indonesia. The BPJS system is divided into two main programs:
1.BPJS Ketenagakerjaan (Employment Social Security), this program protects workers from risks related to their jobs. It covers:
- Work accident insurance
- Life insurance
- Old-age savings
- Pension benefits
- Job loss protection
It is intented for both formal and informal workers including employees, freelancers, business owners, and migrants workers.
2.BPJS Kesehatan (Health Insurance)
This program provides access to comprehensive healthcare service, including:
- Outpatient treatment
- Hospitalization
- Surgeries
- Therapies
- Medicines
BPJS Kesehatan is divided into three classes (1,2, and 3) based on the inpatient room facilities. For class 1 offers more privacy with fewer patients per room, class 2 provides shared rooms with more patients than class 1 but fewer than class 3, and class 3 has the highest number of patients per room but offers the same medical services.
Who Can Register for BPJS?
1.For BPJS Ketenagakerjaan (Employment Social Security), anyone who is working in Indonesia — whether formally employed, self-employeed, or freelance — can register. This includes salaried employees, freelancers, farmers, artist, enterpreneursm, Indonesian migrant workets, untill daily wage laborers.
2.For BPJS Kesehatan (Health Insurance), BPJS Kesehatan is open for everyone with no age restrictions, from newborn babies to senior citizens can become a member as long they provide the required documents.
Can Foreign Nationals join BPJS?
Foreign nationals (non-Indonesian citizens) can also join both BPJS Ketenagakerjaan (Employment Social Security) and BPJS Kesehatan (Health Insurance) under certain conditions. For BPJS Ketenagakerjaan (Employment Social Security) must be employeed in Indonesia for at least 6 months and for BPJS Kesehatan (Health Insurance) must be living in Indonesia for more than 6 months and hold a temporary stay permit or permanent stay permit and a work permit issued by the authorized institution, confirming at least 6 months of employment in Indonesia.
How to Register, Pay, and Claim Benefits?
BPJS Ketenagakerjaan can be registered online through the official BPJS Employment website or offline by visiting the nearest BPJS office. Registration can be done by employers on behalf of their employees, or individuals can register themselves. Payments can be made using the Electronic Payment System, bank transfer, ATM, or the JMO mobile app. To make a claim, participants must meet the eligibility criteria and submit the required documents. Claims can then be processed either online or directly at a BPJS office.

BPJS Kesehatan registration is available online through the official BPJS Health website or offline by visiting the nearest BPJS Health office. It’s important to note that all family members listed on the same Family Card (Kartu Keluarga/KK) must also be registered. Contributions are paid monthly and can be made through both online and offline methods. As of the time of writing, the monthly contribution rates are:
Class 1: Rp. 150.000/person
Class 2: Rp. 100.000/person
Class 3: Rp. 35.000/person
These rates also apply to foreign nationals who are registered in the system. To claim BPJS Kesehatan benefits, participants can go directly to a doctor’s clinic, hospital, or any healthcare facility that accepts BPJS, without the need for separate reimbursement requests.
